Decatur Medi Clinic, your health care provider, creates and uses a patient medical record for health history and related financial information that may be used for:
Continuing care and treatment, a way of communicating with other health care professionals who are involved with patient care, the use of information for billing purposes for patient care, a means of responding to insurance requests for information about patient care, and/or review in quality assessment projects designed to help the clinic improve its ability to provide good health care.
We might collect certain personal information upon patient registration such as name, address, telephone number, social security number, driver's license, insurance identification information, spouse information, guardian information, medical proxy information, allergies, past medical history and employment contact information. We maintain this information in our patient database. We might also collect additional information, such as other family member contact information or emergency contact information.
Our patient's records will be maintained appropriately. Disclosure of a patient's records will not occur without written consent by the patient, except as permitted by law. Release of records consent forms can be obtained from our office at any time. Decatur Medi Clinic's policies about using information might change from time to time as warranted by Health and Human Services Guidelines for Compliance. Patient will be notified at that time for updated information.
Patient's will be allowed to request restrictions on the way their health care information is shared or released by requesting in writing on our patient consent to release form.
Decatur Medi Clinic will maintain technology and methodology that render Protected Health Information Unusable, Unreadable, or Indecipherable to Unauthorized Individuals for Purposes of the Breach Notification Requirements as directed by Human Health Services HITECH / HIPPA compliance act.
